COVID-19 Health Screening/ Liability Waiver:

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.

I further acknowledge that Sincerely Something Borrowed Inc. has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that Sincerely Something Borrowed Inc. cannot guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of

becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Sincerely Something Borrowed Inc. staff, venue staff and other guests present and their families.

I voluntarily seek services provided by Sincerely Something Borrowed Inc. and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.

*I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

* I have not traveled internationally within the last 14 days, and that I do not live with someone who has traveled internationally within the last 14 days. * I have not traveled to a highly impacted area within the United States of America in the last 14 days, and that I do not live with someone who has traveled to a highly impacted area within the last 14 days. * I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

* I have not been diagnosed with Coronavirus/COVID-19 and not yet cleared as non- contagious by state or local public health authorities. * I have not tested positive for Coronavirus/COVID-19 within the last 14 days via PCR or Rapid test, and I do not live with someone who has tested positive via PCR or Rapid test, within the last 14 days. *I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19, such a social distancing, frequent handwashing, and all other CDC and WHO recommendations to mitigate the spread of Coronavirus/COVID-19.

In consideration of reserving services with SSB, Client hereby WAIVES the following claims and lawsuits against SSB its owners, officers, agents, affiliates, successors, employees, independent contractors (collectively "Indemnitees"):

a. Any claims to compensation from Indemnitees, whether under a contract theory or quantum meruit unjust enrichment theory, regarding services for Client where SSB's work was not "completed to likeness". b. Subrogation. Any claims s/he or any other person or entity, or insurer has or may have against Indemnitees that arises out of any death or personal injury suffered by Client or that arises out of any damage to Client's real or personal property. Personal property, includes but is not limited to, any tangible item that is not real estate that is owned, leased, financed, borrowed by, or in the possession of Client.

C. Any claims to recover from Indemnitees Client's attorneys' fees or court costs from and any and all claims for compensatory, or speculative damages. d. To the fullest extent allowed by law, any claims Client has or may have against Indemnitees for punitive, treble and liquidated damages. e. Any claims or causes of action against Indemnitees that arises from catching or the possibility of catching COVID-19 and/or any other contagious bacteria, virus, fungus or infirmity. Client understands and agrees that s/he accepts events from Indemnitees at her/his own risk. f. Any promissory estoppel or detrimental reliance claims against Indemnitees, and damages stemming therefrom for adjustments made regarding Client's events pursuant to or due to termination of this Agreement. g. Client shall indemnify, defend, and save harmless Sincerely Something Borrowed Inc., and its Indemnitees, from and against any and all loss, cost (including attorney fees), damages, expenses, and liability (including statutory liability, general liability, and liability under workers' compensation laws) in conjunction with claims, judgments, damages, penalties, fines, liabilities (including but not limited to personal injury or health), losses, suits, administrative proceedings, and arbitration proceedings arising out of any act or neglect by Sincerely Something Borrowed Inc. and its Indemnitees h. Client hereby releases Sincerely Something Borrowed Inc. of any and all liability or responsibility to the client or any persons (including but not limited to guests listed on contract and vendors) claiming through or under Client by way of subrogation or otherwise for any loss, or damages to individuals or personal property and belongings of client covered by insurance then in force. i. Client is responsible and liable for any injury or illness they obtain or speculate to have obtained during, prior, or post being a recipient of service.

6. In consideration of reserving services for events from Sincerely Something Borrowed Inc. (SSB), Client shall indemnify and hold harmless SSB, its owners, officers, agents, affiliates, successors, employees, independent contractors (collectively "Indemnitees") from any and all third party claims or lawsuits alleging death, bodily injury or property damage, brought by any person or entity against Indemnitees, and Client shall indemnify and hold harmless Indemnitees from any and all judgments rendered against and costs (including attorneys' fees, expert witness costs and court costs) incurred by Indemnitees as result of those third party claims or lawsuits where the death, bodily injury or property damage is caused in whole or in part by: a. Client's and Client's guests, vendors, and independent contractors' work for services performed under this Agreement; b. Client's/Guests' acts or omissions;

C. Vendors' acts or omissions.

d. Client's or Guests' personal property or equipment e. Section 6 in its entirety survives termination of the Agreement.

I hereby release and agree to hold Sincerely Something Borrowed Inc. harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Sincerely Something Borrowed Inc. I understand that this release discharges Sincerely Something Borrowed Inc. from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Sincerely Something Borrowed Inc. This liability waiver and release extends to the salon together with all owners, partners, and employees, contractors and chair-rental stylists/artists.

On this day, I hereby agree to the conditions listed above, that are a requisite of receiving services on the day of my contracted appointments. I certify, to the best of my knowledge, that the above statements are true and accurate.
